I understand, by signing the indemnity form in-person at the gym, that Onyx MMA may be required to collect, use, disclose and/or process personal data (as defined by the Personal Data Protection Act 2012 (“PDPA”) belonging to me and/or the listed minors for the purpose of facilitating and processing class registrations and/or membership applications. In this regard, Onyx MMA will collect, use, disclose and/or process such personal data in accordance with the requiried policies and practices of the PDPA. By providing my personal data above, I hereby consent to the collection, use and/or disclosure of my and/or the listed minors’ personal data by Onyx MMA for the aforementioned purposes.
